After fending off a gang of raiders, I'm planning on having my PCs fight a Dragon Lizard with their newly found relics.
Now, a Dragon Lizard deals 3d20+10 DMG for both of its attack.
BUT, it has a Strength value of 294 which is ridiculous.
So, should I add its modifier to its attack?
Of course, I know that a creature that deals an average of 42 dmg twice each turn is crazy and adding more to it is just downright cruel lol, but I'm asking this question for the sake of clarity.

It was the Dragon Lizard that made me question this too, +60 something damage is "a bit much"
So yeah don't include the modifiers, I'm pretty confident that when we get a split between it a trait and modifier (so in this case STR and DMG) its because the creature needs the trait for something else, like in this case the Repltilus use Dragon Lizards as tanks, so it has to have enough strength to carry a catapult (the one on the weapon list weighs 450KG), The various Tree Squid in the bestiary is like this too, "low damage" but needs insane STR to lift targets of the ground.
Note: I'd totally use the +60 DMG mod is something is prone and Dragon Lizard stood on them.

Hey Marco, welcome to the forums! Glad to see new blood around here.
The usual agreement about monster stat blocks is that they stand as written, i.e. you don't have to add any stat bonuses to those already written. Abe made a Great Point above... sometimes abilities are written to meet an idea, more than to fit the rules exactly. Besides, who wants to do more math than is absolutely necessary?
