Right, so one of my player's characters is about to have a 2nd played birthday. They started at 17 so their first (18th) birthday saw a bunch of stat changes as Table TME-1-10 e.g. his END went up by d20.
now does he get another roll on his 19th birthday or is that it until he's 41. It would seem a bit harsh they get no additional benefits for not dying in the wasteland for a year but the table does make a point of mentioning 100+year olds lose 1 END per year, whereas all the other stat mods are just "-d8"

Well they should be gaining Experience Factors & gaining ranks. Each rank gained grants skill points, END, & a d100 roll on the advancement table. If this was just a survival game then yeah, you probably would gain something for aging, but this is an Post-A adventure game, its all about what you do, not just if you manage to eke out a meager existence for another year. Now RP-wise hell yeah, go splurge a bit & have a banger night out with the team! You might even find another adventure

And also, the benefits from leveling up stack with any benefits (or downgrades) from ageing. Burn the candle at both ends, level up fast, die young! YeeHaaaw!
